Furnishings Request Guide

MMIA considers it a privilege to assist you in furnishing your home.

We thank you for your service in our Armed Forces and wish you the best during this time of transition.

To make a Furnishings request, follow these steps:

  1. Open the HFH Application Document file below

  2. Print out all pages

  3. Read and sign + date the Liability Release And Express Assumption of Risk document

  4. Completely fill out the Furniture Request Form

  5. Make a copy of either your: 

    • DD214 Discharge document*

    • Veteran ID* with photo 

    • ( *DD214 or ID must reflect Honorable, General, or Medical discharge)

  6. Mail the following documents to our address shown below:

    • Furniture Request Form​

    • Liability Release And Express Assumption of Risk document

    • Copy of DD214 -or- Veteran ID
